Because Sunnyside brings together children, teens, and families, we take care in deciding who may participate in our community experiences. This form helps us get to know you, verify the information needed for your application, and make an informed access decision.

Please complete this form with accurate information. If you are a minor, ask your parent or guardian to help you complete the relevant sections and provide the required consent.

Please remember: Completing this form, receiving a family sponsorship, or paying a fee does not guarantee admission. You may attend only after receiving explicit confirmation from Sunnyside for the specific event or activity.

Section 6 — Guest Code of Conduct Acknowledgment

Before continuing, please read the complete [Sunnyside Guest Code of Conduct & Safety Agreement].

The Code explains our expectations concerning Social Grace, kindness, personal boundaries, appropriate content, relationships, privacy, safety, and accountability.
